Orientation to Adult Practice
Orientation to adult practice is the third element in the Six Core Elements of Health Care Transition™ (HCT). The adult practice can begin by identifying clinicians in the practice interested in taking care of new young adults. Young adults are relatively new health care consumers, so it is important to offer an orientation to the adult practice and provide young adult-friendly welcome materials that describe confidentiality, services offered, and the logistics of obtaining care.

Implementation: Orientation to Adult Practice
For each core element, we have created an implementation guide to assist you in establishing a structured transition approach in your practice/clinic/health system. It offers quality improvement (QI) guidance on what information and processes should be considered when undertaking each core element, with examples from different care settings or patient populations.
