The Six Core Elements 3.0 define the basic components of a structured HCT process, which in this package for pediatric, family physicians, and med-peds clinicians, include establishing a transition and care policy/guide, tracking and monitoring progress, administering transition readiness assessments, planning for adult care, transferring to an adult clinician, and confirming completion of transfer.

This HCT approach, recommended in the 2018 AAP/AAFP/ACP Clinical Report, can be customized for clinics/practices/health systems serving youth and parents/caregivers. To implement the Six Core Elements, Got Transition recommends a quality improvement approach to incrementally incorporate the core elements as a standard part of care for youth and their parents/caregivers.
