Got Transition's newest toolkit offers clinicians sample content for a joint telehealth visit to facilitate a "warm handoff" to adult care. The toolkit also includes a sample resource for transferring young adults that explains the telehealth visit.

UPDATED 2021 Transition Coding and Payment Tip Sheet Available
Got Transition and the American Academy of Pediatrics have released their 2021 Transition Coding and Payment Tip Sheet, which includes new and revised CPT codes, current Medicare fees and RVUs for the codes, updated clinical vignettes, and an insurance carrier letter that can be adapted for use to encourage payer recognition of transition-related codes.
